A red, itchy, quarter dollar size annular plaque developed on the left lower leg of a healthy 5-year-old. The lesion cleared after her mother applied hydrocortisone ointment for several days but itching persisted (tinea incognito). A larger lesion recurred at the same site when the corticosteroid ointment was discontinued. Cure was ultimately achieved with topical 1 percent clotrimazole ointment.
