This 76-year-old woman had an itchy round scaly red plaque on the top of her left hand expanding for several weeks. A potassium hydroxide preparation from scale obtained form the plaque revealed hyphae, and a culture grew Trichophyton rubrum. She responded to oral antibungal therapy.
