This 40-year-old woman complained of an itchy scaly red rash on the palms and tops of her hands . A potassium hydroxide preparation of scale obtained from a plaque revealed hyphae, and a culture grew Trichophyton rubrum. She responded to oral and topical antifungal therapy.
