A 67-year-old patient had an erythematous, painful lesion on his glans penis for approximately ten years that was refractory to multiple courses of antibiotics and steroids. This hematoxylin and eosin stained section of the circumcision specimen has the characteristic histologic findings of Zoon balanitis, otherwise known as balanitis circumscripta plasmacellularis.

Description

The epidermis is thin with effacement of the rete ridges, and there is a dense subepidermal plasmacytic infiltrate.
