A 42 year old woman presented to the dental clinic with an extremely tender violaceous plaque with a central draining fistula on the right cheek of 2 months duration. The tenderness was most impressive in the left maxillary premolar area. Four teeth in the area were extracted and an abscess was incised and drained. Unfortunately she developed Streptococcal endocarditis and required 6 weeks of parenteral antibiotic therapy.
