A 33-year-old travelling salesman reported complined of recurrent painful and sometime itchy papules on the back of head just above the hair line. Antibiotics gave only temporary relief. Lesions started insidiously on the nape of the neck and extended to the side. Lesions resolved after epilation laser therapy. He also stopped wearing tight shirts with collars and neck ties.
