This 53 year old man had a 2 year history of minimally pruritic slowly enlarging annular plaques on his neck. Skin biopsy showed granulomatous inflammation with giant cells and histiocytes around fragmented elastic fibers in the upper dermis. Several investigators have proposed that annular elastotic actinic granuloma represents an immune response to weakly antigenic determinants on actinically altered elastic fibers.
