This healthy 6-year-old girl developed a slowly enlarging minimally tender violaceous plaque on her right thigh. A culture from material aspirated from the plaque grow Mycobacterium chelonei. She was treated with clarithromycin for 3 months with some improvement of the lesion. Her sister developed a smaller lesion that never progressed and began to resolve 3 months later without treatment.
