An epithelioid blue nevus is associated with Carney syndrome, but may occur sporadically, as in this case. Some pathologists may consider this a deep penetrating nevus, as it forms a "Y" shape into the dermis and contains a few cells with open vesicular nuclei, but this reflects their many overlapping features. These lesions may look atypical and occasional mitoses may be seen. The symmetry and lack of stromal response are helpful features.

Description

There is a pigmented melanocytic proliferation, confined to the dermis. The lesion fills the papillary dermis and tracks down an adnexal structure into the reticular dermis. Overall, it appears symmetric.
