A 7 year old girl was evaluated for chronic round annular fibrotic plaques with central artophy on her face and extremities since 17 months of age. She had a history of rheumatoid factor negative arthritis, her father had a history of similar arthritis without skin lesions, and she had a younger sister with minimal arthritis and a few chronic cutaneous granulomatous plaques. Skin biopsy showed palisading granulomas in the deep dermis and a more superficial neutrophilic dermatitis.
