This 56-year-old woman had a 2 year history of an extremely pruritic symmetric eruption comprised of multiple, herpetiform, erythematous, papulovesicles on her elbows that rapidly spread to invove the extensor surfaces of her extremities, buttocks, shoulders and posterior scalp and neck. Most of the lesions were excoriated at the time of evaluation. She also complained of bloating and recurrent diarrhea. She responded well to oral dapsone and gluten free diet..
