This 6-year-old boy was evaluated for fever, muscle and joint pain, generalized weakness, and a necrotic psoriasiform eruption on the extensor surfaces of his extremities. He had similar skin lesions with marked edema over his eye lids, mid face, chest, and shoulders. His creatine kinase was >10,000 and magnetic resonance imaging of his legs showed changes typical of dermatomyositis.
