This 34-year-old man immunosuppressed man with leukemia and pancytopenia developed a painful necrotic ulcer with a black eschar on the right side of the back. Blood cultures and cultures from a skin biopsy grew Pseudomonas species.

3 cm oval punched out ulcer with overlying yellowish black crust and surrounding well demarcated erythema
