A 32-year-old man complained of thickened darkly pigmented itchy skin in the groind since age 12. He tried various treatments including antifungal creams, and other topical creams, ointments, and lotions without improvement for many years. Exposure to a desert environment in the Persian Gulf and excessive use of drying soaps resulted in pruritis. A skin biopsy showed changes typical of an epidermal nevus. He refused surgical intervention.
