This 19-year-old Asian man had velvety hyperpigmented plaques involving his back. Interestingly, his brother had a similar condition. He had a normal intellect but suffers from epilepsy. The epidermal nevus syndrome affects the skin, CNS, and skeletal system. The diagnosis is clinical. Histologic findings are fairly nonspecific but include papillomatosis, acanthosis, and hyperkeratosis, as seen in this case. The differential diagnosis would include seborrheic keratoses and perhaps acanthosis nigricans, depending on the clinical setting.

Description

Histologic sections of skin show papillomatosis, mild acanthosis, and hyperorthokeratosis. This is a second lesion from the back.
