A 14 year old boy with acute lymphocytic leukemia who received a bone marrow transplant 35 days earlier developed a violaceous lichenoid eruption on his palms and soles that disseminated to his arms, legs and trunk. The rash was minimally pruritic and resolved with high dose steroids. Note the transverse nail grooves and periungual hyperpigmentation (images of the tops of the hands and feet) from high dose cytoxan received before the transplant.
