A 4 year old boy had a history of recurrent vesicles and pustules on the left palm. Lesions appeared every 4-6 months and cleared over 7-10 days. Bacterial cultures were negative on several occasions. A Tzanck smear demonstrated multinucleated giant cells consistent with Herpes simplex virus infection. He was begun of a trial of oral acyclovir starting at the onset of recurrent infections.
