This is a pretty typical case of Henoch-Schonlein purpura. This child developed a purpuric rash on his lower extremities. He does not have any other known medical issues, but HSP has been associated with systemic findings, specifically renal disease. The histology is of a leukocytoclastic vasculitis. Occasionally eosinophils are present. The diagnosis can be confirmed using direct immunofluorescence studies and demonstrating IgA lodged in vessels.

Description

Small vessels show fibrinoid necrosis of their walls with an intense neutrophilic infiltrate. There is leukocytoclasia and occasional eosinophils.
