A 1 year old boy with fever and a neutrophil chemotaxis defect developed a chronic edematous red plaque with a central crust on the scrotum. Blood cultures grew Pseudomonas and a skin biopsy showed edema, epidermal necrosis and Gram negative rods on tissue Gram stain. There were few neutrophils in the sparse inflammatory infiltrate. The plaque healed with systemic antibiotics.
