This healthy 11-year-old girl developed progressive hyperpigmentation of her right thumb over a year. A punch biopsy showed histologic severe cytologic atypia of melanocytes consistent with melanoma in situ. The lesion was subsequently completely excised. Note the excised matrix and contiguous involved nail bed.
