This 5-year-old healthy girl with incontinentia pigmenti developed a diffuse nail dystrophy of her finger and toe nails. She had some subtle hypopigmented atrophic scars on her arms and legs and several conical and hypoplastic teeth. The nail dystrophy was associated with bony changes of the distal phalanges.
