Cutaneous leishmaniasis is unsual in the United States. However, we have a few cases every year around Denton, TX. The sandfly is the vector and the organism is usually L. braziliensis. The organism packs macrophages giving them a bubbly cytoplasm. The histologic differential diagnosis includes histoplasmosis and toxoplasmosis among others. Leishmaniasis can be diagnosed on H&E sections if one appreciates the kinetoplast. Otherwise a Giemsa stain may be helpful.

Description

Within the dermis, there is a heavy, mixed inflammatory infiltrate. Numerous "punched-out" appearing holes are present. The epidermis is ulcerated (not this section).
