This 68 year old man was evaluated for a large irregularly shaped, variably hyperpigmented patch with scalloped borders that had changed little over the preceding 10 years. A skin biopsy from the center of the patch showed lentigo maligna, and the lesion was excised without complications. Histologic examination of the entire lesion showed a Clark level 3, 1 mm thick lentigo maligna melanoma.
