This 42-year-old man demonstrates the complications of neglected borderline tuberculoid leprosy. Recurrent reactions in this untreated patient led to paralysis of his right hand resulting in clawing and atrophy of hand muscles. Trophic ulcers and gangrene of the middle and ring fingers required amputation.
