This healthy 8-year-old girl developed minimally painful red nodules on her ankles and lower legs 6 months ago. The lesions expanded over months and spread to her thighs. She was treated with oral steroids with rapid clearing of the red nodules but dramatic loss of subcutaneous fat. An exhaustive medical and laboratory evaluation revealed slightly enlarged thyroid gland, decreased thyroid stimulating hormone, normal thyroid hormone, and elevated anti-thyroid antibodies consistent with evolving Hashimoto thyroiditis.

Description

complete loss of fat of both distal lower extremities and partial lipoatrophy of the thighs. Note prominent muscles and vascular structures.
