A 23-year-old farm worker complained of an asymptomatic ulcer on the back of his left elbow for 18 months. It started as a small painless red papule which slowly enlarged by peripheral extension and occasional ulceration with healing in the center. He had no fever, cough, or other systemic symptoms, but a Mantoux test was strogly positive ( more than 7.5 mm) . A skin biopsy showed hyperkeratosis, psedoepitheliomatous hyperplasia, and an infiltrate of lymphocytes, epitheliod cells and Langhan giant cells. No acid fast bacteria could be demonstrated in the tissue sections.
