A 16 year old boy developed a pruritic expanding indurated red plaque 3 days earlier. He also complained of fever, sore throat, headache, and other flu-like symptoms. The plaque continued to grow despite 1 day of oral augmentin and an additional day of oxacillin. In the hospital he developed a lymphangitic streak and left inguinal adenopathy. After 2 doses of doxycycline the lesion began to lighten and flatten. Within 2 days his symptoms resolved. Lyme titers are pending.
