This healthy 37-year-old woman, who lives in an area endemic for Lyme disease, developed an expanding annular plaque on the right supraclavicular area 3 days earlier. She also noted a solitary tender supraclavicular lynph node. 24 hours after starting doxycycline 100 mg twice daily, she developed fever, headache, arthralgias, and myalgias. The plaque and all symptoms resolved 2 days later, and she completed a 3 week course of doxycycline.
