Histopatholigic findings, immunohistochemical stains, and the presence of similar atypical cells in a post auricular lymph node was most consistent with a CD30+ anaplastic large cell lymphoma.
Description
There is a broad area of epidermal ulceration with overlying crust. Adjacent intact epidermis demonstrates irregular epidermal acanthosis with moderate spongiosis and overlying parakeratotic scale. In the underlying dermis, there is a dense perivascular, periadnexal, and interstitial inflammatory infiltrate oomposed of large mononuclear cells with an admixture of plasma cells, neutrophils, and eosinophils. The mononuclear cells demonstrate atypical cytologic features including enlarged nuclei with irregular nuclear contours, prominent nucleoli and a high nuclear to cytoplasmic ratio. Numerous mitotic figures are appreciated. Scattered apoptotic cells are also noted.
