This 54-year-old man with a history of recurrent acne was treated intermittently with oral minocycline. He developed subtle generalized hyperpigmentation and patchy bronze-brown patches on his left leg. He had chronic edema and a well healed scar on the left leg from an old injury. A skin biopsy showed increased melanin and hemosiderin-like pigment consistent with the melanin-iron complex found in some patients with minocycline hyperpigmentation.
