This 38 year old man remembered scraping his knee on a boat while fishing in brackish waters of the upper Chesapeake Bay. Several months later he developed this indurated plaque which persisted for several years. A biopsy showed granulomatous inflammation and a culture grew Mycobacterium marinum. The plaque improved after 3 months of oral minocycline.
