This patient gave a typical history of developing a lesion following a fishing trip. M. marinum may present as a solitary lesion but has been known to have sporotrichoid (lymphatic) spread. In this case, the organsims are not plentiful but readily found on an acid fast stain.

Description

There is an acanthotic epidermis with focal ulceration. In the dermis, there is a mixed inflammatory infiltrate, extending deep.
