This 42-year-old man had a 3 month history of minimally tender swelling of the left index finger. He had a tropical fish tank and was an avid gardener. He was treated with several antibiotics without improvement. A skin biopsy showed abscess formation as well as chronic granulomatous changes. Culture grew Mycobacterium chelonae, and he was treated with doxycycline with gradual improvement.
