This 54-year-old man with myelodysplastic syndrome developed painful ulcerated red plaques on his arms, legs and indurated annular and violaceous plaques studded with pustules and crusts on his chest and neck. Some lesions healed with hyperpigmentation while new lesions continued to appear. Skin biopsies revealed sheets of neutrophils throughout the dermis. Special stains were negative for organisms.
