This woman developed painful nodules on the posterior aspect of her lower extremities over the course of a few months. A subset of these patients will have positive PPD tests (erythema induratum) but this was not the case here. The histologic differential diagnosis would include polyarteritis nodosa, but PAN typically lacks the associated panniculitis.

Description

There is destruction of a medium-sized vessel at the interface of the dermis with the subcutis. The panniculitis involves both the septa and lobules and contains a mixed infiltrate of neutrophils, lymphocytes and histiocytes.
