This 27-year-old sheep farmer developed a painful violaceous plaque on her left thumb a week after being nipped by a ewe. She also developed lymphangitis and regional adenopathy. The lesion was expected to clear without therapy over the subsequent 2 months. A skin biopsy showed the typical changes of Orf.
