A healthy 15 year old girl was concerned when the hyperpigmented papules and plaques that were diagnosed as tinea versicolor did not respond to selenium sulfide lotion and antifungal creams. The lesions on her chest and back were associated with classic velvety hyperpigmented plaques of acanthosis nigricans on the neck, axillae, groin, and bony prominences.
