This 33 year old man was evaluated for chronic painful hemorrhagic crusted eroded lips. He was unable to eat and became cachectic. Multiple cultures for Herpes simplex virus were negative, and several empiric courses of acyclovir produced no improvement. Serologic tests fro syphilis and Human immunodeficiency virus were negative. Skin biopsy and direct immunofluorescence showed changes typical of paraneoplastic pemphigus. A systemic evaluation revealed a large Castleman tumor in his chest. Excision of the tumor resulted in complete remission of the pemphigus.
