This healthy 2-year-old boy developed pain with defecation and decreased appetite in association with a red eroded perianal eruption 6 months ago. Perianal strepcoccal infection was suspected and he was treated with oral augmentin for one week with clearing of the erythema and symptoms within 24 hours. A few days after discontinuing the antibiotic the eruption and symptoms recurred. Augmentin was restarted and continued for 3 weeks with persistent clearing of the infection.
