This is a typical histologic example of a pilar cyst, also called a trichilemmal cyst. This lesion was present on the arm of a 58-year-old woman. Typically, they present on the scalp. The differential diagnosis includes an epidermoid cyst; however, the keratin in the pilar cyst is compact and abruptly forms, without a granular layer.

Description

Histologic sections of skin show a cystic lesion within the dermis. The cyst contains a brightly eosinophilic lining with vaguely papillomatous projections into the lumen. The cyst contents are fluorescent pink with abundant crystalline material.
