A 32-year-old man with Streptococcus viridans subacute bacterial endocarditis had a two-week history of palpable purpura on lower extremities and hematuria. Direct immunofluorescence of the skin biopsy showed IgA, IgG, IgM and complement deposition. Renal biopsy showed crescenteric glomerulonephritis, with absence of IgA deposition.
