A 14 year old girl developed a single red to violaceous scaly 3 cm plaque on her arm followed a week later by multiple smaller papules and plaques disseminated over her trunk and extremities. The rash was minimally pruritic and resolved 3 months later without treatment. The initial plaque is typical of the herald patch that precedes the widespread rash of pityriasis rosea.
