This woman, with a 20 year history of plaque psoriasis, developed diffuse red hyperpigmented papules and plaques with only scattered areas of sparing. She had a prior episode of erythroderma a year earlier that cleared with intramuscular methotrexate and phototherapy with psoralen and ultraviolet A light(PUVA).
