This 50-year-old woman with a history of systemic lupus erythematosus, antiphospholipid syndrome, and urticarial vasculitis developed an enlarging non-healing ulcer on her right ankle 10 months earlier. Multiple tissue cultures were negative, and she failed to respond to oral cyclosporine, plaquenil 200 mg twice daily, cellcept 3 grams/day, and oral prednisone 60 mg/day. Further evaluation failed to reveal infection, and the ulcer began to improve within several days of treatment with pulsed parenteral corticosteroids.
