A 50-year-old woman, with systemic lupus erythematosus and antiphospholipid syndrome, developed progressive painful ulcers for several months. Biopsies showed necrosis and chronic inflammation, and cultures for bacteria, fungi, and mycobacteria were negative. The undermined borders ulcerated, and the lesions eventually healed after aggressive therapy with high dose pulsed steroids an oral cyclosporine.
