This healthy 12-year-old girl had a small portwine birthmark which had become slightly elevated over the last decade. She suddenly developed a friable hemorrhagic papule within the birthmark which bled profusely after minor trauma. The pyogenic granuloma was shaved and cauterized, laser treatment of the portwine birthmark was scheduled. A tendency for pyogenic granulomast to develop within portwine birthmarks has been described in the dermatologic literature.
