The differential diagnosis includes other palisaded granulomatous disorders such as granuloma annulare, necrobiosis lipoidica and rheumatoid nodule. An infectious process should also be excluded, as well as factitial causes.

Description

There is a dermal infiltrate comprised of lymphocytes, histiocytes and occasionally neutrophils. In the upper dermis, this infiltrate is perivascular and nodular, while in the deep dermis and extending into the subcutaneous fat, the infiltrate forms palisaded granulomata. Central necrosis is seen with neutrophils.
