The parents of a 13 month old boy returned at lunch time to find an unconsoleable child with multiple large tense blisters in the diaper area. The penis, scrotum, perinuem, anterior thighs, and perianal area were involved. The posterior buttocks were spared. At the periphery well defined edematous erythema is noted. Investigation revealed that the baby sitter held the boy under a hot tub spigot to teach him a lesson for soiling his diaper. He suffered a full thickness injury of portions of the penis, scrotum, and perineum that required grafting and reconstruction.
