This neutropenic bone marrow transplant patient was evaluated for fever and rash. He had positive blood cultures several days earlier for Gram negative rods, but blood cultures taken the day before evaluation were negative. He was on broad spectrum parenteral antibiotics, but fevers continued to spike to 40 degrees C. Cultures from the skin biopsy also showed Gram negative rods, and the pathology was pending. Although the lesions were widely disseminated, most were aggregated on the extremities.
