This is an unusual lesion because there is a classic cylindroma in the superficial portion of the lesion but this merges with a classic spiradenoma in the deep dermis. These two entities are thought to be related and have been reported as occuring together, prompting some authors to use the term "spiradenocylindroma". They behave in a benign fashion.

Description

The upper panel shows characteristic "blue balls in the dermis". In the lower panel, the basophilic nodules show different cell types, including small basophilic cells, larger vesicular cells, and admixed lymphocytes. Abundant eosinophilic basement membrane material is also noted.
